Fill form with URL that contains values

Our helpdesk system is able to add values from the ticket into a URL that is part of the message. I would like to put the a link into the helpdesk message adding directly the email-address and ticket number that fills out our customer satisfaction form in noteforms.

This would directly fill the email field (4546663c-10ed-4fb7-85ba-634cbd186c37) and the ticket ID (18966b0e-66d6-4f5b-828f-c3aa432d3aa8) field of the form.

PS: we tried to add a script to the form above. It filled out the fields but when sending it of it collided with your own Url Form Prefill feature.
