Scoring feature for questionnaire forms

I have a need to create forms that are questionnaires. The questionnaires would have multiple choice questions and each answer should have a certain number of points set. For example for 1 question, answer A will be 8 points, B will be 6 points, and so on. Then at the end, all the points are added up, and the score is sent (adding or updating) to a a field in an existing record in Notion (using as a CRM).
